We've added a new educational document template to our document generator that can serve as an example affidavit to include in your DS-11 passport application if trying to assert state citizenship.

This template is an example for an affidavit that can be included with the DS-11 passport application. Members may choose to use this when applying for a passport and asserting themselves as a state citizen instead of a US citizen.

We have added an Affidavit of Status to the documents page as concise and easily adaptable written testimony of our status, property, and obligations in relation to the Legal NAME, Trade Names, Social Security, the Legal Identity and our obligations as separate and apart from the obligations of the United States. 

This Affidavit template is a condensed version of our previous POA/DOS template. It is designed to affirm the legal facts between a common law name and identity and the statutorily created Legal NAME & Identity.

This Affidavit of Citizenship template is what many members were asking for as guide to declaring 'American National' status. Anyone int he U.S.A. can utilize it no matter if they are born or naturalized here.

Members may choose to use this document if they feel affirming in written testimony the differences between federal citizenship and state citizenship is necessary.
