When an agency, company, institution, or other organization relies on your identity, records, data, account information, status, or consent, the important question is often not what you intended privately. It is what the written record actually shows.

The Core Rights Affidavit / Notice is a jurisdiction-specific, record-building template designed to help you clearly document your position concerning rights, privacy, conscience, property, data interests, non-waiver, and fair process before or during a dispute.

It is not a legal remedy and does not create rights that the law does not already recognize. Its purpose is simpler: state your position clearly, ask the recipient to identify the authority it relies upon, and preserve an organized written record.    

What This Document Helps You Do

The notice is designed to help you:

  • document your rights-based position in writing;
  • state objections or boundaries before silence is later misunderstood;
  • ask an agency or organization to identify the law, policy, contract, order, consent, or other authority it relies upon;
  • request preservation of relevant records;
  • separate legitimate identification from assumptions about consent or waiver; and
  • create a dated record that can be reviewed later if a dispute develops.

The document may be used by itself or alongside a more specific notice involving privacy, biometrics, medical decisions, employment, education, financial matters, records, agency action, or another defined issue.    

How It Works

You complete the document through the HoMF document generator using your own accurate information.

The Core Rights Affidavit / Notice is generally maintained as the master declaration. A recipient-specific cover letter can then identify who is receiving it, why it is being sent, and what additional documents are enclosed.

What Happens After It Is Sent

The value of the notice is the record it creates.

Keep a copy of the completed notice, cover letter, attachments, delivery information, and any response. If the recipient later replies, refuses, explains its authority, corrects information, or takes further action, those events can be added to the same chronological file.    

The notice does not force a recipient to agree with you, stop acting, or concede a legal issue. It documents what you communicated and what happened afterward.
